Ankylosing spondylitis and three column fracture with Dorsal Myelopathy this 45 year old male presented to us with Back Pain and weakness in legs. He was not able to stand for last few days. His MRI showed classical Anderson lesion with spinal cord compression with cord oedema. He underwent Dorsal laminectomy with pedicles screw fixation. He was mobilised next day and discharge after three days. His neurology and pain has statred showing gradual improvement. Anderson lesion is a three coumn fracture with nonunion seen in Patient with Ankylosing Spondylitis. The only treatment for this is surgery- Pedicle screw fixation otherwise patients develop Neurological deficit as was in this case.
